Registration

To register as a student with a disability, the following forms must be completed each semester the student is requesting services and accommodations:

• Official Request for Accommodations
• Student Responsibility Agreement

If you are a student with a hearing impairment or deafness and desire an interpreter, the following form must be completed each semester you are requesting interpreting services:

• Student Interpretation Agreement

If you are a student with a disability who desires to have a parent, relative, spouse, or other person involved in the decision making process regarding services and accommodations, the following form must be completed:
